And why is it important during the Transition Stage?
First, let’s define the Transition Stage. The Transition Stage is typically when baby is around 3-6 months of age but can extend longer. The Transition Stage is in-between the Swaddle Stage (baby has a strong startle reflex and swaddling provides full suppression of baby’s startle reflex) and the Wearable Blanket Stage (baby able to sleep well in a sleeveless sleeping sack because the startle reflex has faded). The Transition Stage starts when baby first attempts to roll over (typically around 3-4 months but can be sooner), continues to have the startle reflex, and extends until baby’s startle reflex has faded (typically around 4-6 months, can be longer).

Swaddling (typically 0-3 mo but may need to stop as early as 8 weeks) fully suppresses the startle reflex by greatly restricting baby’s movements caused by the startle reflex which reduces awakenings caused by the reflex. Baby’s arms are retained snug, close to body in a swaddle. Some teach to swaddle with arms down by sides. Since 2002 we have taught that limbs should be flexed for proper joint development (per Orthopedists – meaning hips and knees flexed in an abducted position and elbows flexed too) and hands within reach of mouth so baby can self soothe (per multiple infant developmental specialists including Dr. Kathryn Barnard, RN – a famous infant researcher known for Baby Cues). However, snug swaddling must stop as soon as baby shows signs of attempting to roll. Baby must have both arms available and enough arm mobility in the sleep garment they are wearing to be able to push their torso up and reposition their head to get access to air. One arm out and then the other is not a safe technique to use to transition baby from the swaddle if the “attempt to rollover milestone” has occurred, because if baby rolls over with one arm restrained close to body they will not have enough leverage to reposition, and the dreaded Cold Turkey Method of abruptly stopping swaddling and having arms fully free, while safe for a rolling baby, can cause sleep regression for baby and sleep deprivation for parents because when baby doesn’t sleep well, no one sleeps well.
Sleep deprivation is the number one complaint of new parents and sleep deprivation can lead to mental health issues, shaken baby syndrome, marital strife, and poor decision-making when providing a sleep environment for baby. Parents may become so sleep deprived that they choose not to comply with AAP safe sleep recommendations and make risky choices that are known risk factors for SIDS and SUIDS.
The Transition Stage can be a challenging time for unprepared parents and a risky time because the incidence of SIDS and SUIDS peaks during this time when baby is between 2-4 months of age.
Those risky choices and sleep deprivation greatly concerned our founder and she wanted to help parents and babies in the Transition Stage get more sleep and do so safely, so she used her healthcare background as an RN and her design education to create a Purpose-built Transition Swaddle – The SwaddleDesigns Transitional Swaddle Sack®.
The SwaddleDesigns Transitional Swaddle Sack is a patented design with unique half-length sleeves that have mitten cuffs that may be worn open or closed. When both cuffs are closed the sleeves provide partial suppression of the startle reflex, so baby sleeps better, and simultaneously provides enough mobility for baby to use their arms to push up and reposition their head, so baby sleeps safer.
Partial suppression of the startle reflex means baby can move and stretch against the resistance of stretchy fabric of the garment they are wearing. Arms are not retained in one position. Baby can sleep comfortably in their natural sleep position whether their natural position of choice is with arms to out to the side, down, up or on their chest, torso or stomach. The fabric stretches and gives a bit, while supporting baby from wild movements caused by the startle reflex which helps reduce full awakenings, and allows baby to use their arms if they were to roll over. Partial suppression is important for babies who still have a strong Moro reflex, so they sleep better, and the garment must allow enough movement for baby to use their arms if they were to roll over, so baby sleeps safer.

At this photo shoot, from 2018, we were demonstrating that the Transitional Swaddle Sack may be worn under one of our zzZipMe Sleeping Sacks for an extra layer of warmth (layers are recommended by the AAP if more warmth is needed when dressing baby for sleep. No more than one layer more than you would be comfortable in in the same environment). You can see the baby stretch and move their arms in a range of motion, pushing against the stretchy fabric. The fabric provides some resistance. Baby can move their arms up and down, stretch out to the sides, push out forward or bring hand to face for self-comforting. Baby is neither swaddled with arms snug in tight nor can their arms fly out wildly with the startle reflex. You may see baby move their arms around and then settle into their natural position for sleep. You may see baby startle a bit, arouse a bit and then fall back to sleep on their own because they feel their surroundings (which they are used to feeling) feel reassured and safe and fall back to sleep.
